MedStar Health is looking for a Team Leader Community Health Advocate to join our team at MedStar Union Memorial Hospital. We are looking for a Certified Community Health Worker with personal knowledge of the target population, as shown by residency in that neighborhood, cultural competency skills, knowledge of local resources and system navigation, and advocacy and community capacity building skills.
The Team Leader Community Health Advocate will provide front line leadership to the Community Health Advocate team at the assigned facility. Performs the responsibilities of a Community Health Advocate.

Primary Duties:
- Advocates for individual and community health equity. Assists patients in accessing health-related services, including obtaining a medical home, providing instruction on appropriate use, overcoming barriers to obtaining needed medical care and/or social services.
- Assists the Program Coordinator in monitoring performance issues. Contributes to the performance evaluation process by giving feedback to the Program Coordinator/Manager and helping create professional development plans for Community Health Advocate staff members. Monitors and arranges for adequate staffing to ensure appropriate Community Health Advocate coverage for hospital. Monitors daily workflow issues and addresses issues related to same in collaboration with Program Coordination, as necessary. Oversees the orientation of new Community Health Advocate hires by establishing the plan and monitoring progress with other staff, as needed.
- Collects, tracks and reports information about the community and community benefits. Documents all patient encounters; completes and submits monthly reports; maintains comprehensive electronic client files, which include client notes, release of information, assessments and other medical documents acquired on behalf of the patient. Documents activities, service plans, and outcomes achieved by patient in an effective manner.
- Enhances community residents' ability to effectively communicate with health care providers. Integrates with patient care team to support progress in care plan and overall patient wellness. Performs all responsibilities of a Community Health Advocate.
- Identifies departmental performance improvement opportunities and demonstrates a solution-oriented approach in addressing those issues. Manages the team in the Program Coordinator's absence. Keeps Program Coordinator informed about issues related to staffing and problem areas, quality, risk, patient/family issues and concerns, and allocation of resources.
Qualifications:
- High School Diploma or GED.
- 2 years' experience as a community health advocate.
- Certified Community Health Worker in State of Maryland.
- Participation with a community organization, verified by personal or professional reference with personal knowledge of this fact preferred.
- Residency within the target population preferred.
This position has a hiring range of $23.19 - $38.49
